Contributions to LIVESTRONG will specifically support programs and operations of the foundation. Please complete this form and mail your check/money order to the address below:

The earnings generated from your contribution to the LIVESTRONG Foundation Endowment will provide long-term support for the programs operated by the LIVESTRONG Foundation. Please mail your check to the address below:

If you are making an honorary or memorial gift by mail: Please include the family contact name and address (for memorials) or the individual's name and address (for honorary gifts). The Foundation will notify the family or individual of your thoughtfulness but will not reveal the amount of your gift.

Call (877) 236-8820 to make a donation by phone. A LIVESTRONG Foundation staff member will assist you with the process and answer any questions you may have about your contribution.

Securities

We prefer gifts of securities to be made via electronic transfer, rather than mailing actual securities certificates.

If you are giving a security, please verify with your broker that the security is eligible for electronic delivery over the Depository Trust Company (DTC) system. This works for most securities, but some securities have specific transfer instructions.

Please note that we will liquidate all donations of securities as soon as possible. The date of your gift is the date on which your electronic transfer is received into the LIVESTRONG account. The tax-deductible value of your gift is the average share price on the date of your gift multiplied by the number of shares given.

Once you've made a transfer, please contact Kelsey Pullin by phone at 512.279.8371 or by email at giftsofsecurity@LIVESTRONG.org to inform the LIVESTRONG Foundation of the proposed donation and to ensure that you receive a prompt receipt of your gift for your tax records.
